Job Description

The Alzheimer Society of Brant, Haldimand Norfolk, and Hamilton Halton (ASBHNHH) is looking for an Intake & Respite Coordinator. Under the direction of the Team Lead, First Contact this position serves as the initial point of contact and coordinator for all new Behavioural Supports Ontario (BSO) and respite clients of the ASBHNHH.

This position may be primarily located at either our Simcoe or Hamilton Office with the possibility of occasional travel. Office hours are Monday to Friday from 8:30 am to 4:30 pm, but occasional evening work may be required. The hourly rate range for this role is $20.22 - $30.33 per hour.

Tasks:

INTAKE AND REFERRAL:

1. Receives and assigns referrals from Centralized Clinical Intake to the appropriate BSO Community team within the ASBHNHH.

2. Documents information in client database and submits data for tracking.

3. Explores in a sensitive manner the nature of the request for service (i.e. presenting problem, request for service, precipitating factors).

4. Prioritizes cases according to the needs of the client and matches clients with the appropriate program as required.

5. Liaises with other community agencies to facilitate referrals and coordinate client services as required.

6. Speaks with clients on the phone or in person (walk-in), acting as an initial point of contact and information for potential clients.

7. Registers clients for social groups and educational programs, and maintains, updates, and monitors current client waiting lists.

RESPITE COORDINATION:

8. Coordinates respite services for ASBHNHH clients and High Intensity Supports at Home clients in collaboration with the Home and Community Care Support Services (HCCSS) team, Dementia, Resource, Education, Advocacy, Mentorship (DREAM) team and ASBHNHH staff.

9. Reviews and approves invoices for respite services.

10. Submits tracking and enters data into client database, recording demographic information and other client case history and performs statistical reporting monthly, quarterly, and annually.

11. Liaises with Home Care agencies to ensure procurement of appropriate respite services.

12. Updates respite documents and monitors waiting lists as needed.

PROGRAM SUPPORT:

13. Maintains an inventory of both internal and external resources.

14. Attends care review meetings, ASBHNHH staff and branch meetings, and other internal and external committees as required.

OTHER:

15. Represents the Alzheimer Society professionally in the community.

16. Participates in organizational committees and task forces as requested.

17. Complies with organizational policies and procedures.

18. Complies with the duties imposed by law or contract and the policies and procedures for performing the job in a safe and healthy manner.

19. Takes an active role in promoting and protecting personal health and safety and the health and safety of others, both staff and consumers. [Sec. 28(2) OHSA]

20. Performs other duties as assigned.

The above tasks are representative but not all-inclusive.

Specialized Knowledge and Skill Requirements:

  • Experience performing screening and triaging is required.
  • Experience working with persons living with dementia is required along with an in-depth knowledge of Alzheimer’s disease and related dementias.
  • Must have excellent written and oral communication skills; the ability to communicate fluently in French is an asset.
  • Must have the ability to work independently in a professional, self-directed, and collaborative manner.
  • Must have proficiency with Microsoft Office Suite applications.
  • A valid driver’s license and access to a vehicle are required.

Educational Requirements:

  • A university degree in social sciences or a related field, or comparable education, along with knowledge and understanding of community resources and services is required.
  • Membership in a Professional College is preferred.
  • Proficiency in the French language or another 2nd language is an asset.
