Position Title: Clinician – Specialized Geriatric Services (SGS) Rate of Pay: $35.17 - $42.57
Position Status: Temporary Full Time – Approximately 1 year
The Specialized Geriatric Services Program provides non-emergency clinical assessment, consultation, treatment and education to older adults, their families and service providers who are, or know of, someone who is experiencing mental health issues combined with age-related difficulties.
Responsibilities:
Duties include, but are not limited, to:
- Maintains a clinical caseload
- Liaise and provide consultation, treatment, follow up and discharge planning; review and evaluate the treatment and ongoing care in consultation with the client and caregiver/care provider
- Monitor treatment modalities for efficacy and adverse reactions including pharmacological and non- pharmacological interventions
- Complete comprehensive client centered geriatric physical, functional, and cognitive assessments
- Monitor and document client’s progress and response to therapeutic interventions
- Participates in team activities related to clinical care (e.g. statistics) and function
- Participates in activities related to program/organizational development, community/client education
- Participates in clinical coverage duties
- Participates in research within scope
- Involved in group or other treatment modalities as appropriate to patient population and skill level
- Supports, precepts, and aids colleagues, students and others to provide evidence-based best practice clinical service excellence
- This community position provides clinical assessment and support in and across all communities in Haldimand
Norfolk
- Other duties as assigned
Qualifications:
- Baccalaureate level of post-secondary education in Nursing (RN) or Social Worker (BSW/MSW)
- Post graduate certificate in geriatric and/or psychiatric nursing preferred
- Member in good standing with the College of Nurses of Ontario (CNO) or the College of Social Workers and
Social Service Workers (OCSWSSW)
- Minimum 3 years recent related clinical experience with the geriatric population
- Demonstrated sound knowledge and understanding of community mental health client and care giver service needs and treatment modalities
- Demonstrated sound knowledge of theory and practice of geriatric medicine and psychiatry
- Demonstrated clinical acumen, critical thinking and problem-solving skills
- Demonstrated excellent interpersonal and organizational skills
- Demonstrated ability to communicate effectively
- Demonstrated basic understanding of The Mental Health Act of Ontario
- Demonstrated competence with required media equipment (e.g. Computer, Cell phone, etc.)
- Individual must possess a valid driver’s license and a reliable vehicle

Demonstrate proficiency in massage techniques, such as Swedish, deep tissue, and sports massage. Massage therapy: 1 year (preferred).
