Background:

Mint Health+Drugs is looking to hire a Calgary based pharmacist for a full-time hybrid position. The position consists of 40% an on-site community dispensary practice at the Mint Health+Drugs Blue Bottle location, and 60% a work-from-home remote clinical pharmacist services for all Mint locations in Alberta.

A hybrid pharmacist position offers a unique opportunity to blend the best of both worlds - the flexibility of remote work and the personal touch of in-person patient care. With a hybrid role, you can work from the comfort of your home using telehealth technologies to provide medication therapy management and counseling to patients. However, you also maintain the benefits of working in a traditional community pharmacy setting and engaging with patients face-to-face. The role:

Onsite: Mint Health+Drugs Blue Bottle (16-21hrs per week; Mondays and Fridays with occasional weekends). Blue Bottle was established in 1966 as an independent, locally run pharmacy that serves Calgary’s Tuxedo Park neighbourhood and numerous housing/shelter facilities. We’re very much a community pharmacy, with services like our specialized compounding—done entirely in-house—capable of making a huge impact in the lives of our patients. By providing personalized health care to each patient, we’re able to serve as an effective community health care hub.

The hybrid pharmacist position would require on-site dispensary support Monday and Friday each week. Additionally, one weekend per month as per the scheduled rotation. Weekend shifts are short, 3-4 hrs, and offer the flexibility to complete most tasks remotely, only requiring the pharmacist to be onsite to fill urgent orders.

Responsibilities:

Ensure the dispensary operates smoothly and efficiently.

Triage prescription orders and ensure their fulfilment according to established protocols.

Provide pharmaceutical care, including patient assessment, medication therapy management, and drug utilization review.

Mentor and provide guidance to other pharmacy staff, including Pharmacy Technicians and/or Pharmacy Assistants, Pharmacy Students, or Interns.

Uphold all legislation pertaining to pharmacy practice and ensure that the Standards of Practice and the organization's Code of Ethics are followed.

Work collaboratively with other healthcare professionals to optimize patient care.

Maintain accurate and complete patient records.

Remote: Mint Remote Clinical Pharmacist (19-24hrs per week, flexible hours, and workdays). As a remote clinical pharmacist, you will work outside of a traditional clinical setting and provide pharmaceutical care to patients from a remote location. You will be responsible for medication reviews and therapy management, patient education, clinical support, quality assurance, documentation, and telehealth services.

Responsibilities:

Conduct medication review, perform medication reconciliations, and collaborate with healthcare providers to develop and implement medication therapy management plans for patients.

Provide patient education on medication use, potential side effects, and medication adherence.

Provide clinical support to other healthcare providers, such as physicians and nurses, by answering drug information questions and providing clinical decision support.

Monitor and evaluate medication therapy to ensure patient safety and medication effectiveness.

Maintain accurate and complete patient records and documentation of medication therapy management interventions.

Communicate with patients and healthcare providers through telehealth platforms, such as video conferencing and messaging, to provide pharmaceutical care services.

A successful candidate will be a self starter with strong clinical skills, a passion for patient care, and a desire to have a more flexible schedule. The candidate should hold the following credentials:

Education and Experience:

  • Registered pharmacist in good standing with the Alberta College of Pharmacists.
  • 2+ years providing patient care in a clinical setting is preferred.
  • Experience providing clinical services via virtual care.
  • Proficiency in telehealth technologies.

Qualifications and Skills:

  • Comfort in practicing to full scope
  • Skilled in using technology to complete work (typing, remote access, apps etc.) is required.
  • Computer proficiency with Word, Excel spreadsheets, Kroll, Netcare, and other database programs.
  • Strong clinical skills
  • Very strong interpersonal, communication and customer service skills.
  • Accepts and responds to changing priorities.
  • Demonstrated ability to work in a multi-disciplinary team
  • Must have APA certification and be comfortable prescribing.
  • Must be injection certified.
  • Candidates with travel certification (ISTM) and/or experience in conducting travel consults are preferred.
  • Additional certification as a diabetes, and/or respiratory educator is an asset.
  • Desire to work with inner-city populations (addictions/ mental health) and or/ experience is an asset.
  • Additional training and/or certification in opioid dependency is an asset.
